A complete read of the thyroid, paired with the most common non-thyroid causes of the very same symptoms. When you are tired, foggy, or cold, this panel separates a thyroid problem from the iron and B-vitamin shortfalls that feel identical.

Who it is for. Anyone with fatigue, brain fog, or a suspected thyroid issue who wants to test the thyroid and rule out the look-alikes in a single draw.

The core panel

  • TSH The most sensitive screen for an underactive or overactive thyroid.
  • Free T4 The thyroid's main circulating hormone, read alongside TSH.
  • Free T3 The active thyroid hormone, for a fuller read than TSH alone.
  • Thyroid antibodies (TPO + Tg) Flags autoimmune thyroid disease, the most common cause of thyroid trouble. TPO and thyroglobulin antibodies mark Hashimoto's (an underactive thyroid); Graves' (overactive) is confirmed by a different antibody, TRAb.
  • Ferritin Iron stores; a low level is a common, fixable cause of fatigue that mimics an underactive thyroid.
  • Iron + TIBC Serum iron and its binding capacity, separating true iron deficiency from other causes of tiredness.
  • B12 + Folate B-vitamin status; a shortfall causes fatigue and nerve symptoms that overlap with hypothyroidism.

Before testing. Draw in the morning after fasting, since serum iron reads most accurately fasting and can be skewed by a recent iron-rich meal or supplement. If you take biotin, stop it two to three days beforehand, as it can distort thyroid results.

When to retest. Recheck thyroid about 6 to 12 weeks after any change in thyroid medication or dose, and recheck iron or B12 about 12 weeks after starting to correct a deficiency.
